Antenatal & Maternity Services
Local Hospital Self Referrals
Please use the links below for self referral forms to the antenatal services at our local hospitals:
MAT B1 Form
To apply and receive a Mat B1 form, also known as a maternity certificate, please follow the steps below:
- Refer yourself to an antenatal service
- Wait for them to contact us (allow four weeks)
- When you are no more than 20 weeks from the expected delivery date, complete a medical wnquiry form using AccuRx and request a MAT B1.
- We will let you know when MAT B1 is ready for collection.
Sure Start Maternity Grant
You may be entitled to a one off payment of £500
You could get a one-off payment of £500 to help towards the costs of having a child. This is known as a Sure Start Maternity Grant.
You usually qualify for the grant if both of the following apply:
- You’re expecting your first child, or you’re expecting a multiple birth (such as twins) and have children already
- You or your partner already get certain benefits
You must claim the grant within 11 weeks of the baby’s due date or within 6 months after the baby’s birth. You do not have to pay the grant back and it will not affect your other benefits.

Wandsworth Infant feeding Support
Wandsworth infant feeding support services offer evidence-based guidance to help families achieve their feeding goals. We provide support for breastfeeding, maximizing breastmilk, and bottle feeding, including using commercial infant formula safely and responsively.

Homelessness Advice
If you are currently without a home for any reason, please be aware that you are still entitled to register for GP services, even without a fixed address or any proof of address. Article 11.54 of the DHSC Guidelines guarantees you your right to register with us, as long as you are within our practice catchment area. However, please be advised that we must still strictly enforce our catchment area. If you are not resident within our catchment area, we will be happy to direct you to a more suitable surgery.

Shelter is the UK's largest homelessness charity. Their website has useful information and links to resources. They also have an online housing advice service and web chat feature.

Telephone Helpline: 0808 800 4444. Lines are open from 8am to 8pm on weekdays and 9am to 5pm on weekends. You should phone this number if:

For access to psychological therapies services such as CBT, you can refer yourself through the NHS Psychological Therapies service.

Drugs and Alcohol
Wandsworth Community Drugs and Alcohol service offer free and evidence based treatment for people living in Wandsworth who want to change their use of drugs and alcohol.

Eye Problems
The Minor Eye Condition Service is an NHS service available for the assessment and management of minor eye problems in the community. It is run by qualified clinicians such as Opticians or Optometrist’s, and you do not need a GP referral to access the service.
Conditions that they can treat include:
- Red eye or eyelids
- Dry, gritty or uncomfortable eyes
- Eye irritation and inflammation
- A lot of recent discharge from the eye or watery eye
- Recently occurring flashes or floaters
- Painful eye
- Ingrowing lashes
- Recent and sudden loss of vision
- Foreign body in the eye

Muculoskeletal Pain
Get U Better App
Get U Better is an app and website for management and prevention of neck, back, knee, and leg pain. This app is provided free of charge by the Borough of Wandsworth. You don't need to have a referral to use the app - simply follow these steps:
- Visit our surgery's Get U Better link and enter your email address, then select the muscle issue you wish to treat from the drop-down menu.
- Download the Get U Better App and create an account.
- Sign up using the email address which you entered in Step 1, and complete the other field with your personal details.
- Once you have made an account, complete the questionnaire when prompted, and then use the GetUBetter app's advice, tips, and exercises.
Sexual Health

SHWSL offer a full suite of Sexual Health services including all available forms of contraception and STI testing. The nearest clinics are located in Roehampton, Wimbledon and Wandsworth.

NUPAS offers:
