MAT B1 FORM
1. Refer yourself to an antenatal service
2. Wait for them to contact us (allow four weeks)
3. When you are no more than 20 weeks from the expected delivery date, complete a Medical Enquiry form and request a MAT B1.
4. We will let you know when MAT B1 is ready for collection.
